This program, brought to the community by the Alzheimer's Association and Bluegrass Care Navigators, will highlight practical resources that individuals living with dementia and their care partners can access today while also helping them understand the support available as needs change over time. Bluegrass Care Navigators will cover: - Community Engagement and how BCN connects individuals and families with resources- Care Navigation and caregiver support- Advance Care Planning- Palliative Care- Hospice Care- Grief Support services- Volunteer services and opportunities for ongoing connection- Educational programs and community resources available through BCN Alzheimer's Association will cover: - Brain Health- Local resources for support- Nationwide resources for support
